Should I bring anything with me to my first visit?
Just bring a valid photo ID and your payment method for the initial visit. If you have recent lab work, you can bring a copy, but we typically run our own comprehensive hormone panel to ensure accurate results.
What happens at my initial visit?
Your first visit includes a $150 consultation, which is fully credited toward your first month if you begin treatment. During this visit, you’ll meet with James Copley, FNP-C, complete lab work, and review your health history. If you’re a candidate, you may be able to start TRT the same day.
Do you take insurance?
We do not bill insurance. LifeStyle operates on a simple membership model with transparent pricing. This allows us to keep costs affordable and predictable without the restrictions and delays of traditional insurance.
Why pay for a membership if I already have insurance?
Insurance may cover basic labs or visits, but it rarely covers specialized hormone care or TRT. With LifeStyle, your membership gives you direct access, personalized treatment, and full transparency. No hidden fees, no denials, and no surprise bills.
Where is your office located?
We are conveniently located at 5505 W Chandler Blvd, Suite #9 Chandler, AZ 85226. Our office is designed for fast, easy access so you can get care without the long waits or hassles of larger clinics.
Do you offer hormone therapy for men and women?
Yes. While we specialize in testosterone replacement therapy for men, we also provide hormone health solutions for women, including support for perimenopause, menopause, and overall wellness.
How many visits do I get?
With our membership model, visits are included. You’ll have access to as many follow-ups as needed to monitor your treatment, adjust dosages, and ensure you’re getting the best results.
